Registration Requirements

1. Birth Certificate ( Must be State Certified copy)

2. Social Security Card (Cannot accept number written on paper)

3. Immunization Card (Only State approved page will be accepted)

4. Proof of Residence

  • Must have 3 current forms of proof in the Parent/Guardian name only.
  • No Post Office Boxes accepted.
  • Required Documents are as follows: Light bill, gas/butane bill, water bill, satellite/cable bill, vehicle insurance printout, property tax bill, W-2 form, or current year's income tax return.

5. Letter of Assignment (if applicaable)

6. Name, address and/or phone number of previous school

7. Last report card

8. LEAP/Standardize test scores

9. Court Ordered Custody Paers (if applicable)
